Sigarenzakje van papier liggend bedrukt in blauwe inkt in rechthoekige art nouveaulomlijsting links een in een kimono geklede zttende vrouw met haar in een knot, in haar linkerhand een openstaand kistje sigaren met erboven gebogen tekstschild met "S.J. FORTUIN", eronder 'VLAMINGSTRAAT 37, ENGROS-EXPORT" en onder "TELEF. HAAG.3112" langs de onderrand in gebogen tekstschild "'S GRAVENHAGE". Keerzijde liggend bedrukt in rechthoekige dubbele lijst "S.J. FORTUIN, SIGARENHANDEL, VLAMINGSTRAAT NO. 37, DEN HAAG", met eronder "MERK, PRIJS ƒ PER MILLE", langs de onderrand drukkerssignatuur "NADRUK VERBODEN - J. VAN DEN BERG JR".
